After breast augmentation, implants may need to be replaced (e.g. in the case of capsular fibrosis or if you would like a different breast shape after many years).

If your figure changes over time, the shape and size of the implants may no longer fit. Older implants may be defective - or pronounced capsular fibrosis may have developed. The reasons for changing breast implants can be very individual.

What happens during the operation?

Firstly, the planned incision is marked and the planned implant position is marked. Once the anaesthetic has been induced, a local anaesthetic is also injected. The anaesthetic is mixed with a drug that causes the small blood vessels to contract locally. This minimises bleeding during the operation.

After the skin incision, the capsule is opened and the old implant is removed. In the case of defective implants and pronounced capsular fibrosis, the entire capsule must sometimes be removed together with the implant. If it is necessary to change the implant position, the required cavity for the implant is now created. The implant of the desired size and shape is then placed in the breast.

It is often necessary to remove parts of the capsule or sometimes the entire capsule. In some cases, a new pocket for the implants must also be made in order to achieve an optimal result. 

In many cases, we use your old scar as an access point so that no new scars are created. Exact details about a breast implant exchange can only be given after an individual, personal consultation, as your expectations and your previous breast augmentation must be taken into account when planning the operation.

With the patient in a half-sitting position, Dr Kania checks whether the resulting breast shape appears natural and aesthetic. A drain is inserted through which the wound secretions are suctioned out for one to two days. The wound is then closed in several layers with fine sutures. The operation is performed under general anaesthetic and takes about one to two hours.

The modern silicone gel flows downwards when standing and to the side when lying down. This creates a very natural breast shape. Implants filled with saline solution can also be used on request.

FAQ - Frequently asked questions about changing breast implants

What needs to be considered before the implant change operation?

Surgical incisions

The most frequently chosen incisions are in the submammary fold (submammary) or at the lower edge of the areola (periareolar). However, the scars can also be placed through the areola (transareolar) or in the armpit (axillary). As a rule, the existing scars are cut out and used as an access route so that no additional scars are created. Dr Kania will discuss the best procedure for you in detail before the operation.

Placement of the implant

Depending on the requirements, the implant can be placed either partially or completely behind the muscles (submuscular) or between the muscles and the mammary gland (subglandular).

Ultrasound examination, mammography

Dr Kania will discuss with you during the detailed consultation whether an ultrasound examination and/or mammography is advisable before your planned implant change.

What scars can be expected?

As a rule, the existing scars can be excised so that no additional scars occur. Only if the incision was made in the armpit during the first operation will there be an additional scar in the underbust crease or on the nipple.

After three weeks, we start with a scar follow-up treatment. After about six months, the scars usually consist of an inconspicuous thin white line.

What should I look out for after the operation?

In the first few days, it is usually necessary to take painkillers.

The patient can leave the bed on the day of the operation. The inpatient stay is one day. Once the drains have been removed, the special bra must be worn day and night for the next six weeks.

The patient is fit for socialising again as soon as she is discharged from hospital.

Nevertheless, you will need to take it easy physically for the first three weeks after your breast implant replacement. Lifting the arms in particular causes tension on the sutures and should be strictly avoided for the first three weeks. Sports such as tennis and golf or heavy lifting should be avoided for six weeks. The stitches dissolve by themselves and do not need to be removed.

What complications can occur when changing breast implants?

As with any operation, a haematoma or wound infection can occur. Nerves can be injured, particularly when accessing the armpit, which in the worst case can restrict sensitivity to touch. However, this is extremely rare.

The body also forms a capsule around the new implant, which in the vast majority of cases is wafer-thin and does not cause any discomfort. However, a small number of women may experience (renewed) excessive capsule formation (capsular fibrosis).
