Breast reduction Frankfurt - when is a procedure advisable?

In principle, every patient should decide for herself whether she wants to have a breast reduction. 

In our experience, women with large breasts often feel physically or emotionally impaired. The self-esteem of those affected also often suffers. In addition to the overly large, possibly disturbing breasts, affected women complain of discomfort (e.g. neck/back pain or bra straps that cut in).

A breast reduction should only be carried out after the end of physical growth (usually when the woman reaches the age of majority).

Breast reduction can also be of interest if there is a loss of tension in the skin and breast tissue as part of the normal ageing process, after pregnancy or massive weight loss - in this case, a combination with a breast lift is often of interest.

How does breast reduction surgery work?

Before you decide to undergo the procedure, a consultation with our experienced specialist Dr Norbert Kania is necessary. During the individual consultation in our practice in Frankfurt, you will learn everything about the surgical methods, risks, your possible healing process, etc. 

During the procedure itself, the planned incision is first marked with a felt-tip pen while the patient is standing. Once the anaesthetic has been induced, a local anaesthetic is also injected. The anaesthetic is mixed with a drug that causes the small blood vessels to contract locally. This minimises bleeding during the operation.

As a rule, the areola must be reduced and the nipple moved upwards. To do this, the top layer of skin around the nipple is removed according to the previously marked incision without damaging the nerves and blood vessels underneath. The excess skin below the nipple is then removed.

The incision techniques for removing tissue are varied and depend entirely on the patient's needs. The incision depends on how much tissue is to be removed and how much the skin has sagged. A new breast is formed from the remaining tissue, usually with the formation of an "inner bra".

The wound is then closed with continuous sutures, which later dissolve. Drainage tubes ensure that the wound secretions can drain away in the first one to two days after the operation.

The procedure takes two to three hours, depending on the extent and surgical technique.

Type of anaesthesia & hospital stay

Breast reduction surgery is usually performed under general anaesthetic. An inpatient stay is also to be expected (usually 1 to 3 days).

What risks or complications can occur?

As with any surgical procedure, complications can also occur during breast reduction surgery. Your aesthetic surgeon will advise you on your individual risk during your consultation and preliminary examination.

Even if the scars are normally narrow and inconspicuous, a relatively large wound area is created. Possible complications include haematoma and wound healing disorders. If a wound healing disorder occurs due to an infection or circulatory disorder (especially in heavy smokers), wider scars may develop later. These can be corrected around six months later under local anaesthetic.

What needs to be considered before the operation?

Weight loss

You should reach a weight before the operation that you can realistically maintain. The closer you are to your realistic normal weight before the operation, the more satisfied you will be with the result. Significant weight loss after the operation can lead to the breasts sagging again.

Ultrasound examination, mammography

Wound healing

If you are prone to sores in the underbust crease, these areas should be healed before the operation. Your family doctor or dermatologist will help you with this.

Consultation and preliminary examination

Before you decide to undergo your treatment, a comprehensive consultation and individual preliminary examination is necessary. Your doctor will provide you with further information (e.g. the expected course of treatment, possible risks, duration of the operation, details of the costs, etc.). 

What scars can be expected?

The variety of different breast shapes means that there are also different surgical techniques. The scars required can therefore vary. There is almost always a fine scar around the nipple and vertically below the nipple. Dr Kania will discuss with you in detail before the operation whether an additional scar is required in the crease under the breast (like an inverted T or an L).

The best prerequisite for good scar formation is a precise surgical technique. Scar aftercare begins three weeks after the operation in order to achieve optimum scar formation.

What should I look out for after the operation?

In the first few days, it is usually necessary to take painkillers.

The patient can get out of bed on the day of the operation. As soon as the drains have been removed, the special bra or compression bra must be worn day and night for six weeks. Certain sports and heavy lifting should be avoided for six weeks.

The stitches after breast reduction surgery dissolve by themselves and do not need to be removed.

Does health insurance pay for breast reduction?

Most breast reductions are performed for aesthetic reasons. In this case, the costs must be borne by the patient. In the case of medical indications, the costs may be covered by health insurance. However, this is a case-by-case decision. The medical necessity of the treatment must be certified by your attending physician.

If there are medical reasons, you must then apply to your health insurance fund to have the costs covered. The medical service of the health insurance company is often involved in the decision.

What different methods are there for breast reduction?

There are many different methods and names for breast reduction. Here are a few examples:

  • T, anchor or Strömbeck method 
  • I, mushroom, vertical or Lejour method
  • L-method 
  • O or Benelli method

The different methods also differ in terms of the incision (e.g. T-shaped, L-shaped, etc.). The right method for you will depend on your individual physical requirements, e.g:

  • Necessary resizing
  • Individual starting position
  • Expression of skin sagging
  • Are concomitant diseases present?
  • etc.

What should you bear in mind after a breast reduction?

  • Sutures are usually removed within two weeks of the operation. We try to use incisions that leave small scars wherever possible. These are reddened at first - fade over time and become less noticeable.
  • You should not expose your scars to direct sunlight or UV radiation for at least six months. This will reduce the risk of pigmentation disorders.
  • In the first few weeks after the operation, you should not engage in any physically strenuous activities (e.g. sport, etc.).
  • In most cases, the final result of the operation can only be assessed after a few months.
