If the breasts are not only sagging but also very small, a breast lift alone will result in breasts that are firm but much too small in relation to the other proportions. In these cases, a combination of breast lift (mastopexy) and breast augmentation (augmentation) is necessary. These operations can be performed in two stages (first a breast lift and six months later a breast augmentation).

Simultaneous breast lift and breast augmentation (augmentation mastopexy) is one of the greatest challenges for a breast surgeon, as the behaviour of the tissue after the operation has to be virtually calculated in advance.

During the three to four-hour operation, suitable implants are first inserted and then - adapted to the new volume - the breast is lifted and shaped with an "inner bra". The risk of corrective surgery (re-tightening) is greater with augmentation mastopexy than with a breast lift alone.

What happens during the operation?

First, the planned incision is marked with a felt-tip pen while the patient is standing. Once the anaesthetic has been induced, a local anaesthetic is also injected.

The anaesthetic is mixed with a drug that causes the small blood vessels to contract locally. This minimises bleeding during the operation. Suitable implants are then inserted.

After the patient is seated, the original marking is checked and, if necessary, adjusted to the new breast volume.

What happens to the areola?

As a rule, the areola must be reduced in size and the nipple moved upwards. To do this, the top layer of skin around the nipple is removed according to the previously drawn incision pattern without damaging the nerves and blood vessels underneath.

Inner bra & excess skin

An "inner bra" is then formed from the excess skin below the nipple and a new breast shape is modelled from the existing breast tissue around the implants. The wound is then closed with continuous sutures that later dissolve.

Drainage tubes ensure that the wound secretions can drain away in the first one to two days after the operation. The operation is performed under general anaesthetic and takes three to four hours, depending on the extent of the operation.

Choice of implant

The operation should lead to the best possible result, which is why the procedure is preceded by very careful planning. Dr Kania will discuss the various options for breast lifting and augmentation with implants with you.

All implants consist of a silicone shell. We prefer microtextured implants with a minimally roughened surface. The silicone shell is filled with the latest generation of high-quality, cohesive silicone gel that cannot leak.

Modern silicone gels flow downwards when standing and to the side when lying down. This creates a very natural breast shape, as the natural breast behaves in a similar way. Implants filled with saline solution can also be used on request.

Choosing the right implant for you is an important part of surgical planning. It is best to bring a light-coloured T-shirt to the consultation so that Dr Kania can simulate the effect of different implants with you in front of the mirror.

Placement of the implant

The implant is placed either partially or completely behind the muscles (submuscular) or between the muscles and the mammary gland (subglandular).

What needs to be considered before the operation?

Weight loss

Before the operation, you should reach a weight that you can realistically maintain. The closer you are to your realistic normal weight before the operation, the more satisfied you will be with the result. Significant weight loss after the operation can lead to the breasts sagging again.

Ultrasound examination, mammography

Dr Kania will discuss with you during the detailed consultation whether an ultrasound examination and/or mammography is advisable before the operation.

Wound healing

If you are prone to sores in the underbust crease, these areas should be healed before the operation. Your family doctor or dermatologist will help you with this.

Special bra

You will need to wear a special bra for six weeks after the operation. With its wide support surface under the breast and strong lateral support, it will help to shape the breast and ensure that the scars heal as well as possible.

What scars can be expected?

As there are many different breast shapes, there are also different surgical techniques. The scars required can therefore vary. There is almost always a fine scar around the nipple and vertically below the nipple. Dr Kania will discuss with you in detail before the operation whether an additional scar is required in the crease under the breast (like an inverted T or an L).

A precise surgical technique and the skill of the surgeon are the best prerequisites for good scar formation. Scar aftercare begins three weeks after the operation in order to achieve optimum scar formation.

What should I look out for after the operation?

In the first few days, it is usually necessary to take painkillers.

The patient can leave the bed on the day of the operation. The hospital stay is one day. Once the drains have been removed, the special bra must be worn day and night for the next six weeks. Sport and heavy lifting should be avoided for six weeks. The stitches dissolve by themselves and do not need to be removed.

What complications can occur?

The scars are usually narrow and inconspicuous. However, a relatively large wound area is created during the operation. Possible complications include haematoma and wound healing disorders. A usually temporary reduction in sensitivity to touch also occurs occasionally. If a wound healing disorder occurs due to an infection or circulatory disorder (especially in heavy smokers), wider scars may occur later. These scars can be corrected around six months later under local anaesthetic.

The body forms a capsule around each implant, which in the vast majority of cases is wafer-thin and does not cause any discomfort. However, a small number of women may experience excessive capsule formation (capsular fibrosis). This can cause hardening of the breast and, in extreme cases, painful deformation of the breast. In such cases, the capsule must be surgically removed and the implant replaced. The use of implants with a rough surface significantly reduces the rate of capsular fibrosis.

In patients with very lax skin, it can happen that after six months the breast has sagged more than expected. In these cases, a follow-up lift under sedation and local anaesthetic may be necessary.
