Drooping eyelids can be corrected by eyelid surgery.
The change in the upper eyelid commonly known as "drooping eyelids" makes eyes look dull, old and tired. People with this impairment often look years older than they really are. But sunken eyebrows also give the face a tired appearance and can lead to so-called pseudo drooping eyelids, while bags under the eyes and dark circles under the eyes can reinforce this impression. Different scopes of the eyelid lift
Every person is unique and the initial situation for an eyelid lift is always individual. Let's take a look at the structure of an eyelid. It consists of muscles (for lifting, lowering or tightening the eyelid), fat cells, eyelashes and sebaceous and sweat glands. In principle, it is necessary to check which cause is responsible for the development of the visual defect.
A drooping eyelid may be due to excess skin, increased storage of fat cells or stretching or tearing of one of the muscles. Depending on the underlying cause, the surgical method is selected so that the deformity or asymmetry of your eyelids can be corrected with as little risk as possible.
Upper blepharoplasty against drooping upper eyelids / drooping eyelids
An upper blepharoplasty is appropriate for drooping eyelids. Pronounced drooping eyelids can lead to visual impairment (restricted field of vision). Drooping eyebrows often intensify the drooping eyelid (pseudo drooping eyelid).
In modern upper blepharoplasty, muscle fibres and fatty tissue are removed in addition to the excess skin. The worn-out membrane (orbital septum), which is supposed to retain the fat in the eye socket, is tightened. Very mobile, drooping eyebrows that reinforce the drooping eyelid (pseudo drooping eyelid) often have to be fixed with internal sutures.
What happens during upper blepharoplasty?
Before the operation, the planned incision is marked out.
The procedure is performed under local anaesthetic and twilight sleep. An anaesthetic is injected into the surgical area using a very thin needle. The anaesthetic is mixed with a drug that causes the small blood vessels to contract locally. This minimises bleeding during the operation.
During upper blepharoplasty to remove drooping eyelids, muscles and the excess skin hanging down are removed with a scalpel. In most cases, fatty tissue that has protruded from the eye socket over time and caused the drooping eyelids must also be removed. If the eyebrows have drooped, further drooping can be prevented with an internal suture. The skin and muscle are closed with very fine continuous sutures. The operation takes about an hour.
Lower eyelid surgery for bags under the eyes & dark circles
Every person is unique and the initial situation for an eyelid lift is always individual. Therefore, there are different scopes of treatment for eyelid surgery. The outpatient procedure to correct the lower eyelids is often referred to as a lower eyelid lift.
A lower eyelid blepharoplasty is appropriate for bags under the eyes, eye rims and muscle weakness (droopy eye, ectropion). Over the last 20 years, Dr Kania has continued to develop modern surgical techniques for blepharoplasty. Many surgeons only remove the excess skin during upper blepharoplasty. In lower blepharoplasty, often only prolapsed fat and excess skin is removed. As a result, the upper eyelids soon become heavy again as the cause (fat prolapse) has not been removed.
Until the early 2000s, prolapsed fat and excess skin were removed during lower blepharoplasty. Patients initially looked better. With increasing age, however, as more and more fatty tissue is removed from the face, deep-set hollow eyes often developed, resulting in a tired appearance.
In a modern lower eyelid lift (arcus marginalis release with fat transfer), the fat is not removed but shifted downwards to fill the tear trough. Only in cases of extreme excess fat is some fat sometimes removed.
Dr Kania has been performing modern lower blepharoplasty (with "arcus marginalis release") since 2001 and has perfected this technique. The use of modern surgical techniques leads to very good results that usually last a lifetime.
What happens during a lower blepharoplasty?

The procedure is preferably performed under anaesthetic. An anaesthetic is also injected into the surgical area using a very thin needle. The anaesthetic is mixed with a drug that causes the small blood vessels to contract locally. This minimises bleeding during the operation.
Through an incision below the eyelashes, the wafer-thin skin is first detached from the underlying muscle. After opening the muscle, the bony edge of the eye socket is exposed. The worn-out membrane, which is supposed to hold back the fatty tissue in the eye socket, is separated and the fatty tissue is moved to fill the tear trough.
The muscle is then tightened from a small incision on the side of the upper eyelid and finally the excess skin on the lower eyelid is carefully removed. The wounds are closed with the finest continuous sutures. The operation takes about two hours, followed by a day in hospital.
What is the procedure for eyelid surgery?

FAQ - Frequently asked questions about eyelid lifting
What should be considered before an eyelid surgery?
Bring a pair of sunglasses with you to the clinic to conceal the freshly operated eye area and protect it from the wind on the way home. Be aware that the eyes will need to be protected and cooled for the first two days after the operation. Modern lower blepharoplasty with fatty tissue transfer and muscle tightening is an operation that should only be performed by experienced surgeons.
What scars can be expected with an upper blepharoplasty?
The incision is made in such a way that the scars are barely visible later on. After upper blepharoplasty, the scar lies in the fold at the upper edge of the eyeball.
What scars can be expected with lower blepharoplasty?
The scar on the lower edge of the eyelashes and the small scar on the side in the fold of the upper eyelid are usually not visible once the scars have faded.
What to look out for after blepharoplasty?
The eyelids are made up of extremely fine structures and are therefore sensitive to any external influences. You will therefore leave the clinic with slightly swollen eyes. Even the smallest bruises will spread through the delicate tissue, so you may notice a blue discolouration the day after the operation.
Fortunately, the eyes usually recover very quickly. The swelling will subside after just a few days and if a haematoma forms, it will disappear within seven to ten days. You can support the process by cooling and sleeping with your upper body slightly elevated (second pillow) in the first few days after the operation. Decongestant enzymes also help. The stitches are removed after one week.
After about one to two weeks you will be socially acceptable again, with sunglasses or make-up a little sooner.
What complications can occur?
The complications that can occur with any operation (post-operative bleeding, wound infection, excessive scarring, nerve damage) are extremely rare with blepharoplasty and eyebrow lifting. Temporary slight dryness of the eyes may occur at first, but this is usually resolved by moisturising with drops. If too much skin is removed from the upper eyelid, eyelid closure may be impaired. If too much skin is removed from the lower eyelid, the edge of the eyelid may be distorted. However, these are typical problems with surgery performed by an inexperienced surgeon.
What results can be expected from an eyelid lift?
The possible changes depend on your individual starting position and the scope of treatment.
With a Tightening of the lower eyelids (also known as a lower eyelid lift), the aim is usually for the lower eyelids to be wrinkle-free after the operation when you are at rest - but not when you are smiling.
With a Tightening of the upper eyelids (also known as an upper eyelid lift), your upper eyelids will be significantly smoother after the procedure. The aim of this eyelid correction is to open up your gaze and make your eyes look brighter.
Should existing bags under the eyes removed, your eyes will be more alert and open.
